The social service/utilization review department at Jay County Hospital
offers many services to patients and the community. The primary service
provided to our patients is discharge planning. We want to make sure that
when our patients are discharged from the hospital that they have the
services they need to continue to maintain or improve their quality of
life. Referrals to community services may include:
- Home health care
- Home delivered meals
- Lifeline (telephone emergency response system)
- LifeStream services (homemaker, home health aid)
- Hospice care
- Nursing home placement
- Assisted living
- Durable medical equipment
- Referrals to Jay County Hospital Swing Bed Program (short term skilled
nursing care)
- Adoption services
- Information on living wills/health care representative documents
This department is also responsible for providing the clinical information
needed by your insurance company to certify your hospital stay. It is
your responsibility to notify your insurance company of any planned admission
or procedure that may require pre-certification. We will assist you with
notifying your insurance company of any emergency admissions as long as
we have the correct insurance information.
Additional Services:
- Prime Time - A program for those age 55 and Better!
- Seniors Health Insurance Information Program (SHIIP) - This
program offers free informational assistance on Medicare issues
- Jay County Cancer Support Group - This group meets the last
Tuesday of every month from 5:00 p.m. to 6:30 p.m. at Jay County Hospital.
The group is open to anyone experiencing a cancer diagnosis, or friends
and family of a person with cancer
