The social service/utilization review department at Jay County Hospital offers many services to patients and the community. The primary service provided to our patients is discharge planning. We want to make sure that when our patients are discharged from the hospital that they have the services they need to continue to maintain or improve their quality of life. Referrals to community services may include:
• Home healthcare • Home delivered meals • Lifeline (telephone emergency response system) • LifeStream services (homemaker, home health aid) • Hospice care • Nursing home placement • Referrals to Jay County Swing Bed program (short term skilled nursing care) • Adoption services • Information on living wills/health care representative documents
The department is also responsible for providing the clinical information needed by your insurance company to certify your hospital stay. It is your responsibility to notify your insurance company of any planned admission or procedure that may require pre-certification. We will assist you with notifying your insurance company of any emergency admissions as long as we have the correct insurance information.
