We offer a comprehensive outpatient program that consists of an individualized exercise program, cardiac education and risk factor modification.  The program also offers one-on-one interaction with our healthcare staff providing personal and emotional support on the road to recovery.  It is designed for patients with recent heart problems including, but not limited to: 

• Heart Attacks
• Coronary Artery Bypass Surgery (CABG)
• Valve Replacements or Repair
• Angioplasty Procedure
• Stent Placement
• Stable angina
• Coronary Artery Disease
• Stable Heart Failure

cardrehab1.JPG - 4.00 MBOur Cardiac Rehabilitation program is a comprehensive outpatient program monitored by a RN and MD available on the premises.  It consists of three monitored exercise sessions per week for up to 12 weeks.  Heart rate and rhythm are continuously monitored, and blood pressure is measured before, during and after each session.  Educational programs are included, as well as referrals to individually needed resources including dietitian, smoke cessation facilitator, pharmacist, Diabetic Educator.  Weight management, as well as other risk factors that are associated with heart disease is addressed.  A physician prescription is required to begin this program. Many insurance carriers, including Medicare, reimburse for Cardiac Rehabilitation Services. You are advised to contact your insurance carrier to determine coverage amount.

Pulmonary Rehabilitation

Jay County Hospital Pulmonary Rehabilitation is a comprehensive, outpatient program that promotes the restoration of the patient with Pulmonary disease to the fullest health potential that they are capable. It is individually designed to optimize physical and social performance and independence. The program consists of supervised exercise program, education and disease management.

The program is beneficial for patients who have Lung Disease including: Chronic Obstructive Pulmonary disease (COPD), Emphysema, Chronic bronchitis

Pulmonary Rehab is designed to improve the well-being of people who have chronic lung problems. Patients will begin the Pulmonary Rehabilitation program via a referral from their physician. Patients will attend sessions 2 to 3 times per week up to 12 weeks as needed per patient.

Pulmonary Function testing is required before entry into the program.

Pulmonary Rehab offers one on one interaction with our healthcare staff providing personal and emotional support on the road to recovery. Pulmonary Rehab involves a long-term commitment from the patient and a team of health care providers including a physician Medical Director, Registered Nurses and a clinical exercise physiologist. Our collaborating team consists of a Registered Dietitian, Pharmacist, Respiratory Therapist, as well as others as needed. In order to provide complete and comprehensive care, the patient’s physician will be notified of any change in the patient’s status.

Many insurance carriers, including Medicare, reimburse for Pulmonary Rehab services. It is strongly suggested that you contact your insurance company to check your coverage. It is requested that you provide written proof of your insurance and staff can verify your coverage.
